Health reform is law, but the hard work of fixing our health care system is just beginning.

We all deserve comprehensive, coordinated health care, and it’s just not right that the people who are the sickest and most vulnerable are not getting the health care they need.

Doctors should talk to one another — and listen closely to patients’ concerns and those of family caregivers. Providers should work together as a team to better communicate and coordinate our care.

Medical records should be at our fingertips, and patients and their families should not be left to fend for themselves.
